Responsibilities


  • Physical Therapist directs physical therapy treatment toward preventing disability, relieving pain, developing, improving, and/or restoring muscle function, and maintaining maximum performance within the client’s capabilities in accordance with the plan or care/treatment
  • Oversee physical therapy assistants to assure appropriate implementation of the plan of care
  • Evaluates the client’s level of function by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities tests
  • Evaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence of the client
  • Supplies physical therapy services in the home as prescribed by a physician
  • Administers manual therapeutic exercises; instructs, motivates, and assists clients in non-manual exercises and functional activities
  • Administers traction, massage, and treatments involving the application of physical agents
  • Evaluates, fits, and adjusts prosthetic and orthotic devices and recommends any modifications to the physician
  • Participates in the development, review, and updating of the care plan
  • Observes, records, and reports any changes in the client’s condition and response to therapy and makes suggestions for changes in the plan of care to the client’s physician and Case Manager
  • Prepares clinical records (initial evaluations – including OASIS, care plans, progress notes, and discharge summaries) and submits them for the client’s clinical record
  • Participates as a member of the interdisciplinary team in case conferences and in planning and evaluating the total treatment for the client and confers with interdisciplinary team members as needed
  • Advises and consults with the client/family and other agency personnel
  • Submits orders and documentation (clinical and payroll) on a timely basis and according to branch policy
  • Instructs the client/family in the use and care of physical therapy devices and on the client’s total physical therapy program
  • Instructs and supervises Physical Therapy Assistants in certain phases of physical therapy services they may provide to the client
  • Supervises the Physical Therapy Assistant minimally as required by applicable state law
  • Completes an on-site functional assessment, review, and revision of the plan of care and assessment of the utilization of community resources as required by the client
  • Accessible to the Physical Therapy Assistant when a Physical Therapy Assistant is providing treatment to a client
  • Completes other assignments and duties as requested and assigned to accomplish the goals and objectives of the Company


Qualifications


  • Creative thinker who embraces thought leadership
  • Doctorate in Physical Therapy
  • Capable of making challenging decisions and willing to take responsibility for them
  • Graduate of an accredited physical therapy program
  • One (1) year experience as a physical therapist (acute care/home care setting) or as required by law
  • Current PT licensure (in good standing) in the state(s) of practice
  • Current CPR certification
  • Valid driver’s license and automobile insurance

Salary:

$100,000 - $120,000 a year plus full benefits.
