Full-Time Physical Therapists Needed at Right Form Rehabilitation & Wellness Center in Matawan, NJ.

Right Form Rehabilitation & Wellness Center is looking for Doctor of Physical Therapist join our team of health professionals in providing excellent patient care. As Physical Therapist, you are part of a team who works together to maximize outcomes for patients through creative, hospitable, evidence-based care.


Schedule:

Monday – Friday

Flexible Time

40 hours a week

Responsibilities:

  • Provide high quality, safe, compassionate patient care in the acute care setting or the outpatient setting

  • Implements treatment programs to assigned patients, based on the treatment plan established by the physical therapist.

  • Provides evaluation services including review of pertinent history information, the performance of specific assessments, and determination of recommendations and documentation of results.

  • Completes appropriate discharge planning and follow-up care. Accurately and timely documents and charges for patient care activities in light of our evolving health care environment, painting a clear picture of the patient's course of care including focused initial evaluation/plan of care that establishes baseline and goals, legible daily encounter notes, interim progress summaries, and discharge evaluation.

  • Complete and maintain all corporate & clinical services required training and maintain compliance with state/local/federal regulations.

  • Evaluate, treat, and direct treatment for patients for whom physical therapy is medically necessary and document this need clearly.

  • Maintain open and respectful communication with co-workers, physicians, patients, family members, and third-party payors at all times.

  • Attend and/or participate in facility meetings as directed by manager.

Qualifications:

  • Graduate of an accredited school for Physical Therapists

  • Doctorate degree in Physical Therapy (DPT)

  • License required as deemed necessary in the State.

  • American Heart Association's BLS for Healthcare Providers CPR certification required

Salary:
