Responsibilities:

  • Direct physical therapy interventions to prevent disability, alleviate pain, and optimize muscle function, following prescribed care/treatment plans.
  • Supervise and guide physical therapy assistants for precise care plan implementation.
  • Evaluate client functional levels using diagnostic tests, ensuring a holistic assessment of muscle, nerve, joint, and functional abilities.
  • Assess home environments, recommending enhancements for client independence in rehabilitation.
  • Deliver top-notch physical therapy services at the client’s residence as prescribed by physicians.
  • Administer manual therapeutic exercises, guide non-manual exercises, and facilitate functional activities.
  • Provide traction, massage, and cutting-edge treatments with physical agents.
  • Evaluate, fit, and adjust prosthetic and orthotic devices, collaborating on modifications with physicians.
  • Contribute to the development, review, and update of comprehensive care plans.
  • Observe, record, and report client condition changes, making valuable suggestions for care plan modifications.
  • Prepare meticulous clinical records, including initial evaluations (including OASIS), care plans, progress notes, and discharge summaries.
  • Participate actively in interdisciplinary team case conferences, contributing to overall client treatment planning and evaluation.
  • Advise and consult with clients, families, and agency personnel.
  • Submit orders and documentation (clinical and payroll) promptly and in accordance with branch policy.
  • Instruct clients and families on physical therapy device use and comprehensive programs.
  • Mentor and supervise Physical Therapy Assistants in specific phases of their services.
  • Stay accessible for Physical Therapy Assistants when providing stellar treatment to clients.
  • Complete on-site functional assessments, reviewing and revising care plans while assessing community resource utilization as required by clients.

Qualifications:

  • Innovative thinker with a passion for leadership.
  • Ability to make challenging decisions and take accountability for them.
  • Doctor of Physical Therapy (DPT) degree.
  • One (1) year of experience as a physical therapist (in acute care or home care settings, or as mandated by law).
  • Current, in-good-standing PT licensure in the state(s) of practice.
  • Valid CPR certification.
  • Possession of a valid driver’s license and automobile insurance.
  • Strong dedication to clinical excellence, quality, and integrity.
  • Championing the patient care mission with exceptional experiences.
  • Positive attitude and adeptness at collaborative teamwork for patient care.
  • Excellent interpersonal communication and patient/family engagement skills.
  • High energy, focus, work ethic, motivation, and commitment to continuous learning and growth.
  • Proficiency in federal, state, and governing agency regulations.
  • Outstanding presentation, writing, and communication skills.
  • Ability to use professional judgment in delegating selected nursing tasks.
  • Strong capability to be the public face of the organization and serve as an advocate for Home Healthcare, Home Hospice, and other service lines.

Salary:

$90,000 - $100,000 a year plus full benefits.
