Now Hiring: Physical Therapist (PT) – Skilled Nursing Facility (SNF) in Grapevine, Texas
We are seeking a skilled Physical Therapist (PT) to join our Skilled Nursing Facility (SNF) team in Texas. In this role, you will provide individualized therapy services to help residents recover from injuries, manage chronic conditions, and enhance their mobility and quality of life. You will collaborate with a multidisciplinary team to deliver exceptional care in a supportive environment.
Job Type: Full-Time
Work Hours: Monday to Friday, 9 AM – 5 PM
Compensation: $90,000 - $120,000 per year + benefits
What You’ll Do:
- Perform comprehensive assessments to evaluate residents' physical conditions, functional abilities, and therapy needs.
- Develop and implement personalized treatment plans to improve strength, balance, and mobility.
- Administer manual therapy, therapeutic exercises, and rehabilitation modalities (e.g., ultrasound, electrical stimulation).
- Educate residents and caregivers on mobility techniques, fall prevention, and home exercise programs.
- Monitor and document patient progress, adjusting treatments as necessary.
- Collaborate with nurses, physicians, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and compliant clinical records in accordance with HIPAA and SNF regulations.
Requirements:
- Active Physical Therapist license in Texas.
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) from an accredited program.
- Experience in SNF, geriatric, or rehabilitative care preferred.
- Proficiency in therapeutic exercises, mobility training, and patient education.
- Strong communication and interpersonal skills with a patient-first approach.
- Familiarity with electronic health records (EHR) is a plus.
