Job Opportunity: Physical Therapist – Assisted Living Facility
Location: Santa Rosa, California
Employment Type: Full-Time / Part-Time
Schedule: Monday to Friday (9 AM – 5 PM)
Compensation: $90,000 to $100,000 + benefits

Position Overview:

Calling all Physical Therapists (PTs)! We have an immediate opening in an assisted living facility in Colorado. If you have a great understanding of common age-related conditions like arthritis, osteoporosis, dementia, and Parkinson's disease, we encourage you to apply and help us improve our patients' overall quality of life.

Key Responsibilities:

  • Conduct comprehensive physical evaluations to assess range of motion, strength, balance, gait patterns, and functional limitations of each resident
  • Create individualized treatment plans based on assessment findings, including specific goals, exercise routines, and modalities like heat therapy or manual therapy
  • Implement treatment plans through exercises designed to improve strength, flexibility, balance, and coordination, as well as addressing pain management
  • Assist residents with walking techniques, using assistive devices like canes or walkers, and addressing fall prevention strategies
  • Focus on activities of daily living (ADLs) like dressing, bathing, and transferring, ensuring residents can perform them independently or with minimal assistance
  • Instruct residents and caregivers on proper exercise techniques, home exercise programs, and fall prevention strategies
  • Maintain detailed patient records, including progress notes, treatment plans, and functional assessments

Requirements:

  • Valid Physical Therapy license in the state of California
  • Master/Doctor of Physical Therapy degree from an accredited institution
  • Experience in an Assisted Living Facility (preferred but not required)
  • Strong communication and interpersonal skills
  • Ability to develop and implement patient-specific treatment plans
  • Passion for helping patients achieve meaningful recovery
  • Ability to work independently and as part of a collaborative team
