Now Hiring: Physical Therapist (PT) – Skilled Nursing Facility

Salary Range: $100,000 – $120,000 per year

Location: Port Jervis, New York

Job Type: Full-Time

Schedule: Monday to Friday, 9 AM – 5 PM

Our rehab department is equipped with state-of-the-art tools and a spacious therapy gym designed for individual and group sessions—making therapy both effective and engaging.

What You'll Do:

  • Evaluate residents’ physical and functional abilities using clinical assessments
  • Develop and implement individualized care plans tailored to each patient's rehab goals
  • Provide therapeutic interventions to improve strength, balance, gait, mobility, and functional independence
  • Track progress, reassess goals, and adapt treatment plans as needed
  • Educate residents, family members, and caregivers on safe mobility and home exercise techniques
  • Work closely with a dedicated team of PTAs, OTs, speech therapists, nurses, and physicians
  • Maintain documentation in accordance with Medicare/Medicaid and facility requirements

Qualifications:

  • Doctorate or Master’s degree in Physical Therapy from an accredited program
  • Current New York State Physical Therapist License (or eligibility to obtain)
  • Prior experience in skilled nursing, geriatrics, or post-acute rehab is preferred
  • Strong interpersonal and communication skills
  • Knowledge of EMR systems and SNF compliance standards is a plus
