Telehealth Professional Counselor (LCSW, LMFT, LPC)
In furtherance of ensuring that program participants have access to resources that promote their general well-being, advocating for the protection of individual rights and dignity, and enabling people to pursue their own goals, the primary focus of this function will be community stabilization.
Job Schedule : Full-time , Flexible
Your Role :
- Work proactively with participant to avoid use of crisis services
- Complete and maintain crisis alerts for coordination with county crisis services for participants
- Coordinate and document hospital/inpatient service transitions and program transfers when necessary, including care coordination with hospital staff to support discharges.
What We’re Looking For:
- Masters degree in Counseling, Social Work, Marriage and Family Therapist, Mental Health Counseling
- Active license asMarriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW)
- Rapidly establish rapport with a diverse population of patients presenting with a wide range of psychiatric, social and emotional difficulties
- Process and communicate in a manner that is sensitive and appropriate to the cultural, linguistic and psychological needs of patients, families and other healthcare team members.
