Full-time and Part-time Licensed Professional Counselor (LPC) Needed at Veteran's Communicare Therapeutic Clinic.

Our center has expanded into a full-service clinic, including obstetrics, pediatrics, general care, family planning, and dentistry. Furthermore, our highly skilled personnel assist in addressing and managing a variety of social concerns impacting our society. Rest assured that all treatments are provided in the strictest confidence, and we accept Medicaid, Medicare, and the majority of private health insurance policies.

Through therapeutic treatments and good role modeling, the counselor's responsibility include counseling and training patients to improve their emotional, physical, and mental growth. They are expected to reach the program's productivity objective for billable services each week while supporting clients in compliance with approved Agency and Federally Qualified Health Center (FQHC) requirements.

Schedule:

• Full-time and part-time positions are both available
• Hours are based on appointment schedules.

Responsibilities:

• Collaborates with the primary care provider to identify, address, and manage mental health and psychosocial issues, serving as a valuable member of the primary care team.
• Conducts patient assessments, with a focus on diagnostic and functional evaluations, and subsequently offers recommendations to the primary care provider regarding treatment goals and plans for patients.
• Delivers concise, targeted interventions for patients in need of mental health services.
• Provides guidance to the primary care provider in determining which patients are best suited for primary care treatment and which should be referred to specialized mental health facilities or other appropriate resources.
• Initiates follow-up procedures to assess patients' progress and determine the need for adjustments in treatment approaches.
• Develops relapse prevention plans when necessary and supports patients in maintaining stable functioning.
• Assists in identifying "at-risk" patients and formulating plans to prevent the worsening of their conditions.
• Oversees and coordinates the provision of healthcare services for patients in the realm of behavioral health, establishing connections not only within the primary care setting but, with patient consent, also outside it.
• Records patient progress and diagnostic information in the treatment chart.
• Maintains open communication with primary care providers to keep them informed about patients' needs and progress, collaborating on treatment plans.
• Works, as appropriate, to facilitate behavioral changes in patients with physical disorders or those at risk, assisting them in making healthier lifestyle choices.
• Equips patients with self-management skills and educational materials to empower their active participation in their treatment and recovery.
• Offers consultation to clinic management and other team members regarding behavioral services and suggests areas for outcome evaluation and program assessment.
• Aids patients in adhering to any medical treatment initiated by the primary care provider, including strategies to cope with medication side effects.
• Strictly adheres to the Code of Ethical Conduct for Clinical Professionals.
• Undertakes any other assigned duties as necessary.

Qualifications

• Minimum Master’s Degree in Clinical Counseling.
• Current LPC (Licensed Professional Counselor) License
• Knowledge of mental health assessment, treatment planning, developmental psychopathology, systems and methods in family therapy, group therapy, child psychotherapy, DSM-IV diagnoses, and behavioral therapy.
• Bi-lingual English-Spanish speaking proficiency is preferred, but not required.
• Mental and physical health sufficient to meet the demands of the position.
• Thorough knowledge of the personal/social effects of various physical and mental problems on individuals, their families, and the community.
• Considerable knowledge of available community resources and their organization, purpose, and use.
• Knowledge of psychodiagnostics and psychotherapeutic techniques and theories.
• Ability to assess the individual needs of a patient and evaluate the care and services provided to meet those needs.
• Ability to establish and maintain effective, cooperative relationships with professional staff, patients, their families, and other agency and community members.
• Ability to maintain clinical records and prepare related reports.
• Ability to work cooperatively in an interdisciplinary team setting.
• Ability to express ideas effectively.
• Ability to exercise judgment and make independent decisions based on professional knowledge.

Salary:

$87,000-$105,000 per year
