As a counselor at our clinic, you will play an important role in supporting our patients' emotional, physical, and mental well-being. Your duties will involve delivering therapeutic intervention, counseling and education to clients, and maintaining Agency and Federally Qualified Health Center (FQHC) compliance. You will also be expected to fulfill the program's weekly billable service productivity targets. Join our team and help to improve the health and well-being of others.

Schedule:

  • Full-time and part-time positions are both available
  • Hours are based on appointment schedules.

Responsibilities:

- Works with primary care practitioners to detect, treat, and manage mental health and psychosocial issues, adding to the primary care team's overall effectiveness.

- Performs extensive patient intakes, concentrating on diagnostic and functional evaluations, before developing therapy objectives and making treatment recommendations to primary care practitioners.

- Provides those in need of emergency treatment with timely and focused mental health interventions.

- Advises primary care practitioners on which patients are best suited for primary care treatments and which may benefit from specialized mental health services or referrals to relevant experts.

- Conducts regular follow-up evaluations to monitor patients' progress and evaluate the efficacy of treatment options, making modifications as needed.

- When necessary, collaborates with patients to establish relapse prevention methods and supports them.

- Assists in identifying patients at risk of deteriorating health issues and participating in the creation of preventative programs to meet their requirements.

- Coordinates and supervises the delivery of mental health care services to patients, cooperating not just within the primary care context but also, with patient agreement, outside of it, including additional treatment providers as needed.

- Documents patient progress and diagnostic information accurately in the treatment chart, keeping complete and up-to-date records.

- Maintains open contact with primary care providers, ensuring that they are up to date on patients' needs and progress, and actively participates in collaborative treatment planning.

- Administers measures to induce behavioral changes in patients suffering from or at risk of suffering from physical problems, aiding them in choosing better lifestyle choices.

- Provides patients with critical self-management skills and educational tools, allowing them to take an active role in their treatment and recovery.

- Consults clinic management and other team members on behavioral services, identifying opportunities for outcome evaluation and program development.

- Assists patients in sticking to any recommended medical therapy began by the primary care practitioner, including ways for dealing with pharmaceutical side effects.

- Follows the Clinical Professionals' Code of Ethics.

- Performs extra duties as assigned.

Qualifications

- A Master's Degree in Clinical Counseling is required.

- A valid LPC (Licensed Professional Counselor) license is required.

- Understanding of mental health evaluation, treatment planning, developmental psychopathology, family therapy, group therapy, child psychotherapy, DSM-IV diagnoses, and behavioral therapy systems and procedures.

- Fluency in both English and Spanish is encouraged but not needed.

- Sufficient mental and physical health to handle the demands of the position.

- In-depth understanding of the personal/social consequences of different physical and mental illnesses on individuals, their families, and the society.

- Thorough understanding of accessible community resources, including their structure, purpose, and utilization.

- Understanding of psychodiagnostics, as well as psychotherapy approaches and ideas.

- The ability to analyze a patient's particular requirements and evaluate the treatment and services delivered to satisfy those needs.

- The ability to form and maintain successful, collaborative relationships with professional staff, patients, their families, and other agency and community members.

- The ability to keep clinical records and write associated reports.

Salary:

$90,000-$105,000 per year
