Salary

As an LPC, you will play a vital role in providing holistic and patient-centered care by addressing the psychological, emotional, and social aspects of our patients' health. Responsibilities:
  • Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems.
  • Maintains accurate and timely records including clinical and progress notes according to department policy.
  • Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment, and accepted social work interventions.
  • Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems.
  • Assist clients with discharge planning and conducts care coordination. Works closely with various providers in the region or system to facilitate access to services.
  • Consult and coordinate with family and/or support system regarding client treatment and discharge planning.
Qualifications:
  • Master’s degree in Professional Counseling from an accredited program.
  • Current licensure as an LPC or equivalent.
  • Must have at least 3 years of full-time experience in providing mental health direct services
  • Ability to work independently, self-directed and highly organized
  • Proficient computer skills, including MS Office
