Responsibilities:
- Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems.
- Maintains accurate and timely records including clinical and progress notes according to department policy.
- Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment, and accepted social work interventions.
- Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems.
- Assist clients with discharge planning and conducts care coordination. Works closely with various providers in the region or system to facilitate access to services.
- Consult and coordinate with family and/or support system regarding client treatment and discharge planning.
- Master’s degree in Professional Counseling from an accredited program.
- Current licensure as an LPC or equivalent.
- Must have at least 3 years of full-time experience in providing mental health direct services
- Ability to work independently, self-directed and highly organized
- Proficient computer skills, including MS Office
- Annual Salary range of $90,000 - $100,000 per year
