We are seeking employment of a licensed counselor who is eager to work in a diverse environment. This person must have a passion to work in an under-served environment and must be a team player. The individual must believe in the mission and vision of Charter Oak Health Center in order to deliver top customer service.
RESPONSIBILITIES
- Your primary responsibility is to support patients with compassionate care.
- You will conduct assessments of their current state.
- Develop a treatment plan that helps them improve.
- Maintain accurate and prompt patient documentation
- Refer patients for psychiatry services when appropriate.
- Provide feedback and collaborate with the executive team to constantly create a positive patient experience and patient outcomes.
- Master’s degree in Professional Counseling or Psychology..
- Current licensure as LPCC.
- Excellent verbal and written communication skills.
- Demonstrated organizational and planning skills.
- Demonstrates competency in age-related or specialty issues and developmental needs for each population served.
- Experience: A minimum of 3 years of experience is preferred.
