We are seeking a dedicated Licensed Clinical Social Worker as a new member of our team!

Job Summary:

We are seeking an experienced and compassionate Social Worker to join our team. As a Federally Qualified Health Center, we are dedicated to providing quality healthcare services to our community. In this role, you will be responsible for providing counseling services and case management to address the bio-psychosocial needs of our patients.


Qualifications:

Master's Degree in Social Work or a related field is required.

Licensed Clinical Social Worker (LCSW) preferred.

Minimum of two (2) years of experience as a Social Worker with strong interpersonal, oral, and written communication skills. Five (5) years preferred.

Computer literacy, including proficiency in MS Word, Excel, and Outlook.


Responsibilities:

Perform case management and counseling activities to address the bio-psychosocial needs of patients.

Assess patients' psychosocial needs and determine appropriate treatment and referral options.

Assist patients in obtaining and completing forms for entitlements, supplemental insurance, and other benefits.

Advocate for patients when interacting with community agencies.

Maintain accurate and up-to-date patient records.

Coordinate transportation assistance for patients when necessary.

Attend case conferences and team meetings.

Identify the need for patient education and support services.

Conduct need assessments and education for prenatal and postpartum patients.

Consult with the Provider and make appropriate referrals when necessary.

Evaluate the needs of special population patients.

Provide HIV pre-test and post-test counseling and testing.

Follow up on missed appointments with patients.

Participate in and contribute to Quality Improvement and Quality Assurance activities.

Maintain a clean and safe work area.

Perform other relevant duties as assigned or requested.

**Job Summary:**

The Mental Health Counselor will be responsible for performing case management and counseling activities related to the bio-psychosocial needs affecting the health and well-being of Center patients.


**Duties and Responsibilities:**

Assess patient’s psychosocial needs and determine appropriate treatment/referral needs, including case management short-term counseling and crisis intervention.

Assist patients with obtaining and completing forms for entitlements, supplemental insurance, and other benefits that will help eliminate dependence on indigent and emergency assistance programs.

Help patients access prescription cost assistance through indigent programs sponsored by pharmaceutical companies and community resources.

Act as an advocate for patients when interacting with community agencies.

Maintain up-to-date and accurate patient records, prepare reports, records, and other written documentation as needed.

Coordinate patient’s transportation assistance when necessary.

Attend clinical and team meetings related to case conferences.

Identify the need for patient education and support services such as pregnancy counseling, parenting skills, caregiver skills, etc.

Conduct need assessments and education for prenatal and post-partum patients.

Consult with the Provider and make appropriate referrals when necessary.

Evaluate the needs of special population patients, including the homeless, substance abusers, and at-risk adolescents.

Provide HIV pre-test, post-test counseling, and testing as needed.

Follow-up on missed appointments with patients who have a pattern of excessive cancellations or failure to show.

Participate in and contribute to Quality Improvement and Quality Assurance activities and functions as needed.

Maintain a clean, safe, and orderly work area.

Attend meetings as required.

Regular attendance is required.

Perform other relevant duties as assigned or requested.


**Qualifications:**

Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), or Licensed Mental Health Counselor (LMHC). NYS Permit holder is encouraged to apply.

Three (3) years of experience as a Mental Health Counselor or Social Worker with strong interpersonal, oral, and written communication skills.

Experience with trauma-informed care and EMDR Therapy is preferred.

Computer literacy and proficiency in MS Word, Excel, and Outlook.
