Job Summary:


We are seeking an experienced and compassionate Licensed Mental Health Counselor to join our team. As a Federally Qualified Health Center, we are dedicated to providing quality healthcare services to our community. In this role, you will be responsible for providing counseling services and case management to address the bio-psychosocial needs of our patients.


Qualifications:

  • A Master's Degree in Social Work or a related field is required.
  • Licensed Mental Health Counselor (LMHC) preferred.
  • Minimum of two (2) years of experience as a Social Worker with strong interpersonal, oral, and written communication skills. Five (5) years preferred.
  • Computer literacy, including proficiency in MS Word, Excel, and Outlook.

Responsibilities:

  • Perform case management and counseling activities to address the bio-psychosocial needs of patients.
  • Assess patients' psychosocial needs and determine appropriate treatment and referral options.
  • Assist patients in obtaining and completing forms for entitlements, supplemental insurance, and other benefits.
  • Advocate for patients when interacting with community agencies.
  • Maintain accurate and up-to-date patient records.
  • Coordinate transportation assistance for patients when necessary.
  • Attend case conferences and team meetings.
  • Identify the need for patient education and support services.
  • Conduct assessments and education for prenatal and postpartum patients.
  • Consult with the Provider and make appropriate referrals when necessary.
  • Evaluate the needs of special population patients.
  • Provide HIV pre-test and post-test counseling and testing.
  • Follow up on missed appointments with patients.
  • Participate in and contribute to Quality Improvement and Quality Assurance activities.
  • Maintain a clean and safe work area.
  • Perform other relevant duties as assigned or requested.

Physical Demands:

This position requires periods of sitting, standing, walking, bending, and stooping. Weekend and evening work may be required.


Salary Range:

$115k to $130k a year
