Full-time Licensed Medical Social Worker (LMHC, LMFT,LICSW)

The Medical Social Worker is responsible for providing consultation and service to homebound persons who require aid in resolving complex social and emotional concerns that interfere with medical regimen maintenance.

Schedule: Full Time

What You Will Do:

  • Initially performs a comprehensive assessment of the patient and caregiver needs and formulates appropriate plan of care.
  • Establishes specific goals and regularly evaluates progress toward these goals.
  • Demonstrates ability to identify and is sensitive to the comprehensive needs of patients and families.
  • Continually evaluates patient's needs and modifies care plans appropriately in collaboration with patient and caregiver.
  • Develops a plan of care determined by the assessment process with goals established for the appropriate outcomes in collaboration with the physician.
  • Implements a plan of care in collaboration with patient and caregiver.
  • Assists the physician and other health team members in understanding the significant social and emotional factors related to a patient's health problems.
  • Assesses and reassess the social and emotional factors to estimate the patient's capacity and potential to cope with problems of daily living.
  • Assists patients and their families with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.
  • Consults with other health personnel regarding patient social and emotional problems.
  • Participates in in-service education and activities of PI program and follow up as assigned.
  • Performs other assignments as requested.

Minimum Qualifications:

  • Master's Degree from a school of social work accredited by the Council on Social Work Education
  • Social Work License Required per State Regulations.(LMHC, LMFT,LICSW)
  • Ability to meet occupational requirements of the position including the following: Ability to maintain and project a mature, professional, and courteous demeanor to patient, families, staff, health care professionals and external constituents.
