Full-time Home Care Social Worker Needed for Home Care Agency in Norfolk, Virginia

Work Hours:

  • Monday to Friday: 8:00 AM to 4:30 PM
  • Occasional evening or weekend hours may be required based on patient needs and availability.
  • Flexibility in scheduling is necessary to accommodate patients' home care visits and ensure continuity of care.
  • On-call availability may be required to address urgent situations or provide support during off-hours.

Salary:

$60,000 to $80,000 per year

Job Duties:

  • Conduct comprehensive assessments of seniors' psychosocial needs, including emotional, mental, and social factors.
  • Develop individualized care plans in collaboration with clients and their families, addressing their specific goals and preferences.
  • Provide counseling and emotional support to seniors and their families, assisting them in coping with life transitions and challenges.
  • Advocate for clients' rights and access to community resources, ensuring they receive appropriate support and services.
  • Collaborate with interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate and enhance care.
  • Conduct regular home visits to monitor clients' well-being, assess their living conditions, and address any concerns or safety issues.
  • Assist in facilitating communication and resolving conflicts among family members involved in the care of the senior client.
  • Educate clients and their families about available community resources, support groups, and programs to enhance their social engagement and overall quality of life.
  • Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and any relevant interactions.
  • Stay informed about current social work practices, policies, and regulations, and ensure compliance with ethical standards.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Social Worker (LCSW, LMSW, or equivalent).
  • Minimum of two years of experience in a social work role, preferably in a home care or geriatric setting.
  • Strong knowledge of social work principles, aging-related issues, and community resources.
  • Excellent assessment, counseling, and problem-solving skills.
  • Empathy, compassion, and the ability to build rapport with seniors and their families.
  • Strong communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
  • Proficiency in using electronic health record (EHR) systems and computer applications.
