We specialize in physical therapy, occupational therapy, and speech-language pathology services
to assist patients in regaining mobility, improving functional abilities, and achieving their highest
level of independence. Our team of rehabilitation professionals collaborates to deliver
individualized, goal-oriented care.

You will contribute to the overall well-being of our patients by addressing their emotional, social,

Work Schedule:
Full-time position with regular business hours, including weekdays, with occasional evening and
weekend shifts based on patient needs and assigned caseloads.
Responsibilities:
-Conduct psychosocial assessments through interviews, observations, and review of
medical records to evaluate patients' emotional, social, and environmental factors
impacting their recovery.
-Provide counseling and support to patients and their families, addressing adjustment
issues and emotional well-being.
-Collaborate with the rehabilitation team to develop individualized treatment plans,
considering patients' psychosocial needs.
-Assist patients in accessing community resources, such as support groups, vocational
rehabilitation, or housing assistance.
-Provide support and education to patients and families regarding community resources
and post-rehabilitation services.
-Participate in discharge planning and facilitate transitions to home or other care settings.

-Active state licensure as a Licensed Clinical Social Worker (LCSW).
-Experience in a rehabilitation or medical setting is preferred.
-Strong understanding of adjustment issues, grief, and coping mechanisms.
-Excellent communication and interpersonal skills.
Salary:
$75,000-100,000 plus bonus
