As a Social Worker, you will play a vital role in evaluating the psychosocial needs of our clients and coordinating services to enhance their overall well-being.

Work Hours:

  • Monday to Friday: 8:00 AM to 4:30 PM (Full-time schedule)
  • Occasional evening or weekend hours may be required based on client needs and availability.
  • Flexibility in scheduling is necessary to accommodate visits and ensure continuous care.
  • On-call availability may be required to address urgent situations or provide support during off-hours.
  • Part-time schedule is based on needs and work hours are negotiable.

Job Responsibilities:

  • Conduct thorough assessments of seniors' psychosocial needs, considering emotional, mental, and social factors.
  • Collaborate with clients and their families to develop personalized care plans addressing their specific goals and preferences.
  • Offer counseling and emotional support to seniors and their families, helping them navigate life transitions and challenges.
  • Advocate for clients' rights and access to community resources, ensuring they receive appropriate support and services.
  • Work alongside interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate and enhance care.
  • Conduct regular visits to monitor clients' well-being, assess their living conditions, and address any concerns or safety issues.
  • Facilitate communication and resolve conflicts among family members involved in the senior client's care.
  • Educate clients and their families about available community resources, support groups, and programs to enhance their social engagement and overall quality of life.
  • Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and relevant interactions.
  • Stay informed about current social work practices, policies, and regulations, ensuring compliance with ethical standards.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Social Worker (LCSW)
  • Minimum of two years of experience in a social work role, preferably in a community center or geriatric setting.
  • Thorough knowledge of social work principles, aging-related issues, and available community resources.
  • Excellent assessment, counseling, and problem-solving skills.
  • Ability to empathize, show compassion, and establish rapport with seniors and their families.
  • Strong communication and interpersonal skills, enabling effective collaboration with interdisciplinary teams.
  • Proficiency in using electronic health record (EHR) systems and computer applications.

Pay:

$70,000 to $100,000 per year
