Work Schedule:
Full-time position with a flexible schedule, including day, evening, and weekend shifts based on departmental needs and assigned rotations. On-call responsibilities may be required.

Job Responsibilities:
Conduct comprehensive psychosocial assessments and collaborate with medical teams for holistic patient care.
Conduct psychosocial assessments to identify patients' emotional, social, and financial needs.
Provide individual and group counseling to patients, addressing adjustment, grief, and coping issues.
Coordinate with community organizations to arrange post-discharge services, ensuring continuity of care.
Offer emotional support and education to patients and families regarding treatment options and resources.
Participate in hospital committees and provide expertise on social work-related matters.
Provide crisis intervention, counseling, and emotional support to patients and their families.
Develop and implement discharge plans and connect patients with appropriate community resources.
Collaborate with interdisciplinary teams to optimize patient outcomes and improve patient satisfaction.
Maintain accurate and timely documentation in compliance with hospital policies and legal requirements.

Qualifications:
Master's degree in Social Work from an accredited program.
Active state licensure as a Licensed Clinical Social Worker (LCSW).
Two years of experience in a healthcare or hospital setting preferred.
Knowledge of medical terminology, mental health disorders, and crisis intervention techniques.
Strong communication, interpersonal, and collaboration skills.

Salary:
$70,000-$100,000 + bonus, depending on experience and expertise
