You will provide therapeutic interventions, support, and advocacy for our patients, and contribute to their recovery working with a team of dedicated professionals.

$70,000 - $100,00 + Bonus


Responsibilities:


  • Conduct comprehensive psychosocial assessments and collaborate with medical teams for holistic patient care.
  • Conduct psychosocial assessments to identify patients' emotional, social, and financial needs.
  • Provide individual and group counseling to patients, addressing adjustment, grief, and coping issues.
  • Coordinate with community organizations to arrange post-discharge services, ensuring continuity of care.
  • Offer emotional support and education to patients and families regarding treatment options and resources.
  • Participate in hospital committees and provide expertise on social work-related matters.
  • Provide crisis intervention, counseling, and emotional support to patients and their families.
  • Develop and implement discharge plans and connect patients with appropriate community resources.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and improve patient satisfaction.
  • Maintain accurate and timely documentation in compliance with hospital policies and legal requirements.


Requirements:


  • Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong knowledge of mental health disorders, treatment modalities, and crisis intervention.
  • Ability to work collaboratively in a multidisciplinary team.
