  8. Conduct brief, evidence-based screenings and arrange detailed assessments when needed.
  9. Create and implement integrated care plans, ensuring access to linked services.
  10. Provide brief, focused prevention, treatment, and recovery services for clients.

Duties & Responsibilities:

  1. Work directly with assigned clients in both office and community settings.
  2. Collaborate actively with the team to achieve shared goals.
  3. Communicate respectfully and effectively with individuals from diverse backgrounds.
  4. Navigate organizational and financial structures effectively.
  5. Adhere to professional ethical guidelines and COHC's code of conduct.
  6. Collaborate to enhance skills and achieve performance goals.
  7. Identify, prevent, and report unsafe situations.
  8. Conduct screenings and arrange detailed assessments when necessary.
  9. Create and implement integrated care plans.
  10. Provide brief, focused prevention, treatment, and recovery services for clients.
  11. Assist with crisis intervention efforts as needed.
  12. Manage a caseload, including assessment and various therapy modalities.
  13. Monitor client meetings, admission, continued stay, and discharge criteria.
  14. Ensure timely completion of reports and data submission.
  15. Participate in committees to enhance agency service delivery and quality improvement.

Qualifications:

  1. Strong expertise in assessing, diagnosing, and treating assigned populations, along with risk assessment and crisis management.
  2. Excellent verbal and written communication skills.
  3. Demonstrated organizational and planning skills.
  4. Competency in addressing age-related or specialty issues and developmental needs for each population served.
  5. Preferred minimum of 2 years of clinical experience.
  6. Master's degree or Doctorate in Social Work.
  7. Required licensure as LCSW in the state.
  8. Bilingual in English/Spanish is preferred.
