Salary: $120,000 to $130,000 per year

Job description

The Licensed Clinical Social Worker is responsible for the provision of primary care medical services with a focus on access to care and responsible for the direct provision of behavioral health services meeting all established productivity expectations.

Responsibilities:

• As an ancillary health team member, participate in managing the care of patients/ families though individual visits and group classes.
• Promotes and participates in daily huddles.
• Responsible for setting and monitoring self- management plans.
• Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
• Completes chart and peer review responsibility in a timely manner.

Qualifications:

• Master’s degree in Social Work (LCSW) from an accredited institution.
• Valid LCSW license in California.
• Proven experience as a LCSW minimum of 2 Years
• Demonstrated skills in the clinical mental health services with individuals, families, and groups.

• Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
