SALARY
$120,000 to $130,000 per year with benefits
RESPONSIBILITIES
- Engages patient and family in goals of care conversations and advance care planning
- Balances business needs with patient advocacy
- Works directly in the office with assigned clients who experience symptoms of major mental illness and/or severe emotional disorders
- Provides a range of brief, focused prevention, treatment, and recovery services for clients
- Attends and actively participates in care team meetings
- Builds solid working relationships with internal team members, matrix partners, key functional areas, customers and providers
- Provides decision support where multiple treatment options are available
- Empowers customers with skills to provide enhanced interaction with their providers
- Master’s degree in Social Work or Counseling.
- Valid licensure as LCSW n applicable state required.
- Expert knowledge of assessment, diagnosis, and treatment for assigned populations, risk assessment, and crisis management.
- Minimum of 3 years of experience is required.
- Ability to navigate in a fast-paced working environment
- Prioritizes work to meet commitments aligned with organizational goals.
