Looking for a full-time Licensed Clinical Social Worker (LCSW) for a job opportunity.

At our clinic, creating a safe and therapeutic environment is our priority. As Licensed Clinical Social Worker you will primarily work closely with a diverse group of professionals who are committed to delivering exceptional mental health care. This will offer you the opportunity to gain clinical experience and cultural competence while serving clients from various backgrounds.

Schedule: MONDAY - FRIDAY (9:00AM-5:00PM)

Salary: $90,000 to $100,000 per year plus bonus

Key Responsibilities Are:

  • Conduct comprehensive psychosocial assessments of patients with to identify needs, strengths, and challenges
  • Provide exceptional counseling, disease management and health education to patients
  • Develop and implement personalized care plans in collaboration with patients and an interdisciplinary healthcare team, focusing on behavior modification, coping strategies, and lifestyle changes
  • Support goal setting for individual patients synchronously and asynchronously to help them better manage their chronic conditions and associated mental health needs
  • Conduct regular outreach to patients, based on their needs and preferences, to support engagement and elicit behavior change
  • Document patient interactions, progress, and care plans accurately and promptly in the electronic health record (EHR) system

What We Need:

  • Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of 3 years of experience in a mental health or outpatient setting.
  • Strong knowledge of evidence-based therapeutic modalities and treatment approaches.
  • Excellent interpersonal, communication, and assessment skills.
