We are currently seeking a Social Worker to join our team to provide one-on-one case management services to clients with substance use disorder diagnoses. This position assists clients with managing chemical dependency issues and promoting efforts for education and recovery. This position will be responsible for reviewing progress of the clients and participate in trainings to further enhance their professional focus on mental health and substance use disorder clients in a residential setting.
SALARY $120,000 to $130,000 per year with benefits
RESPONSIBILITIES
- Provide social work services to patients and their families
- Conduct assessments to determine the psychosocial needs of patients
- Develop and implement care plans based on patient needs and goals
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Collaborate with multidisciplinary teams, including pediatricians, to address the behavioral health needs of clients.
- Document patient interactions and maintain accurate records
- Master’s degree in Social Work, Counseling or a related field.
- Current licensure as LCSW.
- Minimum 3 years of experience working with children, families, and individuals with developmental disabilities.
- Anticipates and adopts new technology and provides feedback to drive innovation.
- Ability to recognize, address and resolve conflicts in a professional, collaborative manner.
