SALARY: $90,000 - $110,000 per year and benefits

Key Responsibilities:

• Complete thorough assessments and create individualized care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Use therapeutic interventions to support clients in achieving emotional and behavioral stability.
• Document all clinical activities accurately in the electronic health record system.
• Advocate for clients’ needs and connect them with community resources.

REQUIREMENTS:

• Master's degree in Social Work from an accredited institution.
• Must hold a valid LCSW license in the state.
• Must have experience working with older adults
• Strong clinical and diagnostic skills
• Excellent communication and interpersonal skills
• Must be able to work independently and as part of a team
