Primary Responsibilities:
• Perform virtual psychosocial assessments to understand client needs
• Provide therapy to individuals and groups using evidence-based practices
• Collaborate with healthcare providers to develop comprehensive care plans
• Advocate for clients’ access to community resources and services
• Maintain clear, accurate documentation of client interactions and progress
• Contribute to team meetings and ongoing professional development

What We Require:
• Master’s degree in Social Work
• Active LCSW licensure
• Experience with geriatric clients is preferred
• Strong clinical, diagnostic, and communication skills
• Ability to work independently while supporting a collaborative team
