Job Details:

• Type: Full-time
• Salary Range: $110,000–$130,000 annually

Key Responsibilities:

• Perform biopsychosocial assessments tailored to patients' age and cultural backgrounds in primary care settings.
• Deliver counseling services to individuals, couples, and families, including crisis intervention and treatment planning.
• Act as a liaison to connect patients with local resources and community programs.
• Support quality improvement initiatives by contributing to data collection processes.

Requirements:

• Master’s degree in Social Work, Counseling, or a related field.
• Active California LCSW license.
• At least 2 years of clinical service experience.
• Proficiency in MS Office and electronic health records.
