Work Hours: Monday to Friday, 9 AM to 5 PM

Responsibilities:

• Provide critical support to patients managing mental health and social complexities.
• Partner with interdisciplinary teams to ensure coordinated care delivery.
• Build networks with local resources to address patients’ unique needs.

Salary: $90,000 - $110,000 per year, plus comprehensive benefits.

Requirements:

• Master’s degree in Social Work or a related field.
• Active LCSW certification.
• Prior experience in geriatrics or elderly care.
• Skilled in diagnostic assessments and individualized care planning.
• Excellent interpersonal and teamwork skills.
