Full-Time Licensed Clinical Social Worker (LCSW) Opportunity in Fremont, California

We are looking for a skilled Licensed Clinical Social Worker (LCSW) for a full-time position in California. This opportunity will allow you to apply your skills in a dynamic healthcare setting. You will be responsible for performing mental health evaluations, documenting progress, and conducting suicide risk assessments, as well as escalating cases to providers as needed.

Job Type: Full-time
Work Hours: Monday to Friday, 9:00 am – 5:00 pm

Responsibilities:

• Conduct comprehensive biopsychosocial evaluations for mental health, substance use, and medical concerns in a primary care context.
• Provide behavioral health interventions to individuals, families, and couples, including counseling, diagnostic assessments, treatment planning, crisis intervention, and referrals for substance abuse treatment.
• Facilitate referrals and provide social work support, including connecting patients with local resources and assistance programs.
• Partner with the Quality Improvement team to develop and implement data collection processes.

Salary: $110,000 – $130,000 per year

Requirements:

• Master’s degree in Social Work, Counseling, or a related field.
• Active LCSW license in the state of California.
• Minimum of 2 years’ experience providing clinical services.
• Proficient in MS Outlook, Word, and other computer-based programs.
• Capable of completing documentation in an electronic health record (EHR) system.
