Licensed Clinical Social Worker (LCSW) – Full-Time Role in Pomona, California

Are you a Licensed Clinical Social Worker (LCSW) looking for a full-time position in California? This role includes responsibilities such as documenting progress, assessing suicide risks, and coordinating patient care with appropriate healthcare providers.

Job Type: Full-time

Work Hours: Monday to Friday, 9:00 am – 5:00 pm

Responsibilities:

• Provide biopsychosocial assessments addressing mental health, substance abuse, and medical needs in a primary care setting.
• Deliver behavioral health services, including counseling, intake assessments, treatment planning, crisis intervention, and referrals for substance abuse treatment.
• Offer social work support and assist with referrals to community resources, including medication assistance programs.
• Collaborate with Quality Improvement teams to ensure effective data collection processes are in place.

Salary: $110,000 – $130,000 per year

Requirements:

• Master’s degree in Social Work, Counseling, or related discipline.
• LCSW license in California.
• 2 years of experience providing clinical services.
• Proficiency in MS Outlook, Word, and similar software.
• Experience documenting in electronic health record systems.
